IMPRESSION : CT findings are suggestive of: 

• Moderate collection is noted in peritoneum in perihepatic, perisplenic and bilateral paracolic gutter and pelvic cavity with internal loculations with enhancing peritoneum --- suggestive of pyoperitoneum. 

• Fluid-filled dilated bowel loops seen in peritoneal cavity few of them showing air fluid level predominantly involve small bowel loops – likely early SAIO changes. 

• Liver is enlarged measuring approx. 19 cm in size. --- Hepatomegaly 

Spleen is mildly enlarged in size, measuring approx. 13cm – mild splenomegaly.
